Because there were already two bicuspids. In fact, this patient had his/her two bicuspids, but had lost an incisor and a fang. There was space for only one of these lost teeth, so the dentist decided to restore the patient's own bicuspids and put an incisor in, in an attempt to preserve the best possible symmetry. If the patient smiles, you would see the four incisors and the firs bicuspid could pass as the fang.
